Classic ReBoot applications are available in two formats:
ONLINE APPLICATION
A two-part process especially for those completing the process on their mobile devices:
PARTS 1-4: Follow the link below to complete Part I to IV of the online form. Each part must be done in one sitting (information cannot be saved).
PART 5: At the end of completing Part IV you'll receive instructions on completing Part V and submitting remaining documentation via email at reboot@outreach.ca
ADOBE APPLICATION
Download the pdf version of the application and fill it out at your own pace at home before submitting it via Email. Ensure that you are able to:
- download the form and enter your information,
- save the form with your entries,
- email the completed form via email to reboot@outreach.ca.

ADOBE ACROBAT DOWNLOAD
In order to fill out the pdf version of the application you must have Adobe Acrobat Reader installed (click here to download), also please make sure to un-check the "optional offer", McAfee. Open the downloaded PDF in this program and proceed to fill out the form as required, save the filled out form, and email it to reboot@outreach.ca.
